Individuals may enter the sweepstakes by answering the questions in the survey and supplying their email address following the last question. Entries may also be submitted by delivering a legibly written entry with your name, mailing address and phone number to a Personal Banker at the Lake Mills or Watertown location or by mail to: Bank of Lake Mills Attn: Marketing, 136 E. Madison St., Lake Mills, WI 53551 (the “alternative entry option”). Odds of winning are the same for those who complete the survey and those who enter via the alternative entry options. Customers and non-customers of Bank of Lake Mills (BLM) are eligible. Winners will be randomly drawn by BLM personnel. Only one entry may be submitted per person. No purchase is necessary to enter or win. Must be 18 years of age or older and a legal resident of Wisconsin. Entries will be accepted until 11:59PM CST on 09/30/2023. Actual odds of winning depend upon the total number of eligible entries. BLM directors, employees, and immediate family members are not eligible. BLM does not sell names or other personal information to third parties. On or around 10/13/2023, winners will be notified. By entering the sweepstakes and accepting the prize, winners agree to use of their name and likeness in Bank of Lake Mills' marketing materials. Other restrictions may apply.
